Authorization Letter for Medical Reports: Free Format

An authorization letter for medical reports is a short signed note in which a patient permits another person to collect their lab reports, scans or hospital records on their behalf. It must name both people, list the reports, give the date, and carry the patient's signature. The person collecting should carry the letter, a copy of the patient's ID proof and their own original ID. Most labs and hospitals in India accept this simple format.
Copy-paste formats you can use today
Your mother had a thyroid test at Dr Lal PathLabs. You're at work. Your brother is near the lab. Instead of letting the report wait, write a two-minute letter and send him. Here are three formats that cover most situations.

What the collector must carry, and the rules at labs vs hospitals
Labs usually hand over a printed report with a simple letter. Hospitals are stricter, because a full case file is far more sensitive than one blood test.
Situation | Letter needed | Patient's ID | Collector's ID | Extra documents |
Lab test report (Dr Lal PathLabs, SRL, Metropolis, Thyrocare) | Yes, or booking SMS plus ID | Copy | Original | Bill or booking ID |
Hospital discharge summary | Yes | Copy | Original | UHID or IP number |
Full hospital case file | Yes, signed by patient | Self-attested copy | Original | MRD form, fee receipt |
Imaging CD (X-ray, CT, MRI) | Yes | Copy | Original | Radiology receipt |
Deceased patient's records | Request letter from relative | Death certificate copy | Original | Proof of relationship |
Patient unconscious or a minor | Letter from guardian or next of kin | Patient's ID copy | Original | Relationship proof, doctor's note if needed |
Why hospitals check carefully
Medical records contain private information. India's Digital Personal Data Protection Act, 2023 and the NMC's confidentiality rules make hospitals cautious about sharing them with the wrong person. A clear authorization letter for medical reports protects the patient and makes the staff's job easy.
Consent letter for medical records vs authorization
People use these terms loosely. A consent letter lets someone see or receive your information, for example an insurer. An authorization letter lets someone act for you, such as collecting reports. For records collection, an authorization letter for medical reports, like the authorisation letter sample India formats above, is what MRD staff expect.
In simple terms: The letter says "I allow this person to pick up my reports." The hospital checks that the letter, your ID copy and their ID all match. If everything matches, they hand over the papers. |
Special cases and mistakes that get letters rejected

When the patient can't sign
If a parent is unwell, a thumb impression with a witness signature is commonly accepted. For an unconscious patient, the spouse or next of kin usually applies with relationship proof and, sometimes, a note from the treating doctor. Minors' records are collected by a parent or legal guardian. In each case, an authorization letter for medical reports signed by the next of kin, with relationship proof, is the usual starting point.
Frequent reasons for rejection
• Patient name doesn't match the hospital registration spelling
• No date, or the letter is several months old
• Missing UHID, IP number or booking ID
• Collector's original ID left at home
• Letter asks for "all records" with no hospital stay dates
• Letter signed by the collector instead of the patient
Make the request part of a bigger plan
An authorization letter for medical reports solves one trip. To build a lasting system, read our pillar guide on how to get medical records from hospital, including the five-working-day rule. If you're collecting records because the family is moving, our guide to transfer medical records to a new doctor covers the next step.
Seht tip Once your brother collects the report, scan it into your mother's Seht profile right away. Next time, the doctor can see it without anyone making another trip. |
When to see a doctor
Collecting reports should never delay care. Contact the doctor promptly if:
• A report is marked "critical" or the lab calls you about it
• Results show very high sugar, very low haemoglobin or abnormal kidney values
• The patient has new fever, breathlessness or chest pain while waiting
• A biopsy or cancer marker report is ready, and you haven't had it explained
• The patient seems confused or much weaker than usual
In an emergency, call 112 or 108 straight away. Reports can follow; care cannot wait.
FAQs
How do I write an authorization letter for medical reports?
Write the date, the lab or hospital's name, and a subject line. State the patient's name, booking or UHID number, and authorise a named person, with their relation and ID number, to collect specific reports. Add the patient's signature and attach a self-attested ID copy. The collector must carry their own original ID.
Can someone else collect my blood test report?
Yes. Most labs, including Dr Lal PathLabs, SRL and Metropolis, release reports to a person who brings an authorisation letter, the patient's ID copy and their own original ID. Many labs also send reports by SMS, email or app, so check whether a digital copy is already available before sending someone.
Is an authorization letter for medical reports needed for family members?
Usually yes, even for a spouse or adult child, because hospitals must protect patient privacy. Some labs accept the booking SMS plus ID for close family. Hospitals almost always ask for a signed letter for case files. For minors, parents can collect without a letter by showing their own ID and relationship proof.
Does an authorization letter need to be on stamp paper or notarised?
No, not for collecting reports. A plain-paper letter signed by the patient, with ID copies, is accepted by most labs and hospitals in India. Stamp paper or notarisation is only needed for legal documents like a power of attorney. Some hospitals may ask you to fill in their own form as well.
How do I collect medical records of a deceased parent?
Write a request to the hospital's Medical Records Officer as the legal heir or close relative. Attach the death certificate, proof of relationship and your ID. Ask for the death summary and complete case file, and state the purpose. Hospitals usually release certified copies within a few working days after verification.
