To link health records to ABHA, you either let connected providers send records to your account or upload them yourself. When you give your ABHA number or @abdm address at an ABDM-enabled hospital, lab or pharmacy, records created there can be linked to your ABHA once you approve. For old prescriptions, paper reports or private clinic slips, you scan or upload them into an ABDM app. Over 105 crore records had been linked to ABHA accounts by July 2026.

Route 1: automatic linking from ABDM-enabled providers

This is the cleanest route. The hospital or lab creates a digital record, and it becomes available in your ABHA app. Nothing to scan, nothing to type.

Step by step at a hospital or lab
  1. Ask at registration: "Are you ABDM-enabled?" Large government hospitals and many private chains are.

  2. Give your ABHA number or @abdm address, or use Scan and Share at the counter.

  3. Complete your visit, test or admission as usual.

  4. Open the ABHA app or Aarogya Setu 2.0. Look for a notification to link records from that facility.

  5. Approve with an OTP or in-app confirmation. The record now appears in your account.

Link lab reports to ABHA after a test

To link lab reports to ABHA, share your ABHA at sample collection, not after the report is out. Labs connected to ABDM tag the report to your account when they publish it. If you've already received a report without linking, open your ABHA app, search for the lab under "link records", and request discovery. Under the government's Digital Health Incentive Scheme, labs and pharmacies have already been paid ₹2.95 crore for linking records, so many are motivated to help.

Route 2: upload records yourself

Most Indian families have years of paper: a thyroid report from 2019, Dadi's angiography CD, a child's allergy prescription. These won't auto-link. You add them yourself.

Upload prescription to ABHA in five steps
  1. Open the ABHA app or Aarogya Setu 2.0 and select the right family profile.

  2. Go to the health locker or records section and tap Upload or Add record.

  3. Take a clear photo or pick a PDF from your phone.

  4. Choose the record type, such as prescription or lab report, and set the date.

  5. Save. Aarogya Setu 2.0 can also digitise the scan using AI.

That's how to upload prescription to ABHA. Repeat for each important document.

What to upload first

Don't try to digitise everything in one evening. Prioritise:

•     Discharge summaries from any hospital admission.

•     Reports for long-term conditions, such as HbA1c, thyroid or kidney function.

•     Allergy notes and current medicine lists.

•     Imaging reports, such as ECG, echo and MRI findings.

•     Children's vaccination cards.

Your ABHA health locker isn't a cloud drive

Your ABHA health locker in a PHR app holds self-uploaded files, but it's not unlimited storage for every bill and scan. Keep it focused on documents a new doctor would actually need.

IN SIMPLE TERMS

In simple terms: hospitals and labs that are part of the government system can send your reports to your ABHA once you say yes. For older papers, you take a photo and add it to the app yourself. Either way, your records end up in one place a doctor can see with your permission.

How ABDM consent keeps you in control

Linking a record doesn't make it public. Understanding ABDM consent is what lets you link health records to ABHA without worry.

Records stay where they were created

ABDM doesn't copy your files into one central database. A report stays with the lab or hospital that made it, stored under their own retention rules. Your ABHA acts as an address book pointing to it. When a new doctor wants your history, they send a consent request. You approve it, choose the time period and choose the record types.

Revoking access

Consent is time-bound and revocable. You can see active consents in your app and withdraw them any time. Want the full picture on data safety? Our explainer on is ABHA safe covers encryption, consent managers and India's DPDP rules.

When something's linked to the wrong account

If you spot a record that isn't yours, don't approve it. Report it to the facility and to the ABDM helpline at 1800-11-4477. If you've decided you don't want an ABHA at all, our guide on how to delete ABHA account explains what happens to linked records.

WATCH OUT

Watch out: Never approve a consent request you don't recognise, even if it shows a hospital's name. Confirm with the facility first.

When to see a doctor (and when to check your records)

Linking records helps doctors, but it's not a diagnosis. Book a consultation when:

  • A newly linked report shows values marked high or low.

  • An old and a new report show a big change, such as rising HbA1c or creatinine.

  • A discharge summary lists a follow-up date you've missed.

  • A child's vaccination record shows overdue doses.

  • Medicine names on a new prescription clash with an allergy note in your records.

In an emergency, go to the nearest hospital or call 112 or 108. Doctors will treat you first and look at records later.

FAQs

How do I link health records to ABHA?

You can link health records to ABHA in two ways. At ABDM-enabled hospitals, labs and pharmacies, share your ABHA at registration and approve the record when it appears in your app. For older documents, upload photos or PDFs into the health locker of the ABHA app or Aarogya Setu 2.0. Both methods are free and quick.

Yes, if the lab is ABDM-enabled. Ask at sample collection and share your ABHA number or @abdm address. The report links when it's ready, after you approve. If the lab isn't connected, download the PDF report and upload it to your ABHA health locker yourself, so it still sits with your other records.

Uploading through official or ABDM-listed apps uses OTP login and consent-based sharing. Doctors can't see your records unless you approve a consent request, and you can revoke access later. Keep your phone locked, never share OTPs, and upload only to official apps such as the ABHA app or Aarogya Setu 2.0.

Records appear only from ABDM-enabled facilities, and usually only if you shared your ABHA during that visit. Older visits may not be linked. Try the "link records" or facility search option in your app and request discovery. If nothing shows up, ask the hospital's records department for copies and upload them manually.

No. A doctor needs your consent each time. You choose which record types and which time period to share, and the access expires. Your records stay with the hospital or lab that created them, and ABHA only connects them. You can review and withdraw active consents in your app at any time.
