What TSH Is and Why the Thyroid Runs on It

TSH, or Thyroid Stimulating Hormone, isn't actually made by your thyroid gland it's released by the pituitary gland in your brain to tell the thyroid how much hormone to produce. This makes TSH an indirect but remarkably sensitive way to check thyroid function, which is why it's almost always the first test ordered.

The pituitary-thyroid feedback loop, explained simply

Think of it as a thermostat. When thyroid hormone runs low, the pituitary raises TSH to push the thyroid to work harder. When thyroid hormone runs high, the pituitary drops TSH to slow things down. A high TSH therefore usually means an underactive thyroid, and a low TSH usually means an overactive one which can feel backwards the first time you hear it.

TSH vs T3 and T4 which test comes first

TSH is the standard first screen precisely because it changes before T3 or T4 levels do, catching thyroid dysfunction earlier. If TSH comes back abnormal, doctors then typically add free T4, and occasionally free T3, to understand the full picture before starting any treatment.

Below 2.5

ICMR-recommended tighter cut-off

Pregnancy — 2nd / 3rd trimester

Below 0.4

Suggests hyperthyroidism (overactive)

Above 4.5 – 5.0

Suggests hypothyroidism (underactive)

The standard adult range and what pushes it

Values just outside the 0.4–4.0 range are common and don't automatically mean disease stress, recent illness, certain medications, and evenx the time of day can shift TSH slightly. It's a pattern across repeat tests, not one isolated reading, that actually guides treatment decisions.

Why pregnancy has its own thyroid rules

Thyroid hormone is essential for a baby's early brain development, and the body's thyroid demand rises noticeably in the first trimester. That's why the acceptable TSH ceiling drops during pregnancy a level that would be unremarkable outside pregnancy can prompt treatment during it.

In simple terms: TSH tells your doctor whether your thyroid is working too little, too much, or just right. A number between roughly 0.4 and 4.0 mIU/L is typically fine for most adults, but pregnancy shifts that range lower. One TSH reading is a snapshot a pattern across two or three tests tells the real story.

Why Thyroid Testing Matters So Much for Indian Women

Thyroid disorders run several times more common in women than men, and hypothyroidism in particular is widespread enough in India that many gynaecologists now check TSH routinely at antenatal visits and postpartum follow-ups, rather than waiting for symptoms to appear.

Who should be screened, and how often
  • Women above 35 screening every 5 years, even without symptoms

  • Anyone planning a pregnancy, or already pregnant

  • People with Type 1 diabetes or another autoimmune condition

  • Anyone with a family history of thyroid disease

Test cost and where to get it done

A standalone TSH test costs roughly ₹150–400 at private labs across India, with discounted online bookings occasionally bringing it under ₹150. A full thyroid profile covering T3, T4 and TSH together typically runs ₹400–800, available through home collection from Dr Lal PathLabs, SRL, Metropolis, or Thyrocare without needing a prescription.

TSH Alongside the Rest of Your Reports

Thyroid hormone influences liver enzyme levels in ways most people don't expect untreated hypothyroidism can mildly raise the numbers on an lft normal range india report, which is one reason doctors sometimes order both tests together when fatigue is the main complaint.

The liver-thyroid connection few people know about

Thyroid hormone affects how the liver processes fat and cholesterol, so an underactive thyroid can quietly nudge liver enzymes and lipid levels upward. This is usually reversible once the thyroid is treated, but it means a liver report shouldn't always be read in isolation from a thyroid one.

Why tracking the trend matters more than one report

TSH is also typically drawn alongside a routine cbc test normal range india check, since both fatigue and anaemia can mimic thyroid symptoms. Comparing reports side by side across visits rather than reacting to a single number is what actually helps a doctor tell the two apart.

When to See a Doctor

  • TSH consistently above 4.5–5.0 mIU/L on two separate tests

  • TSH below 0.4 mIU/L, especially with a racing heartbeat

  • Unexplained weight change, fatigue, or hair thinning alongside an abnormal TSH

  • A TSH above 2.5 mIU/L during the first trimester of pregnancy

  • A visible swelling or lump near the front of the neck

Seek emergency care for a very rapid heartbeat, chest pain, high fever, or confusion in someone with known thyroid disease this can indicate a thyroid storm, a rare but serious emergency.

FAQs

What is the normal TSH range for Indian adults?

Most Indian labs consider TSH normal between 0.4 and 4.0 mIU/L for adults, though some extend the upper limit to 4.5 or 5.0 mIU/L. Always compare your result to the reference range printed on your own report, since the exact cut-off can vary slightly by lab and testing method.

A high TSH usually means your thyroid isn't producing enough hormone on its own, so the pituitary gland releases more TSH to push it. This is called hypothyroidism, and it's typically confirmed alongside a free T4 test. Common symptoms include fatigue, weight gain, and cold intolerance, though many people have no symptoms at all early on.

TSH is generally kept below 2.5 mIU/L during the first trimester of pregnancy, slightly higher than that in the second and third trimesters. ICMR recommends trimester-specific ranges rather than the standard adult range, since even mildly elevated TSH during pregnancy is treated more cautiously than it would be otherwise.

A standalone TSH test typically costs ₹150–400 at private labs, with discounted online bookings from chains like Thyrocare sometimes bringing it under ₹150. A full thyroid profile that includes T3 and T4 alongside TSH usually costs ₹400–800, and no fasting is required for either.

Women above 35 are generally advised to screen every five years if there's no known thyroid issue, and more often with symptoms, pregnancy, or a family history. Anyone already on thyroid medication is typically tested every 6–8 weeks after a dose change, then every 6–12 months once stable.
