The main documents required for hospital admission in India are the patient's photo ID (Aadhaar, PAN, passport or voter ID), a doctor's admission note or referral, past medical records and the current medicine list. For cashless treatment, add the health insurance e-card, policy number and the attendant's ID. For government schemes, add the Ayushman card or ABHA. Keeping these in one folder, physical and digital, can cut admission time from hours to minutes.

The hospital admission checklist: what to carry

It is midnight. Your mother needs admission for pneumonia. The registration desk asks for ID, insurance details, previous reports and a deposit, while you're also talking to doctors. A ready folder of documents required for hospital admission turns a stressful hour into a ten-minute task.

Medical documents that change the treatment

Doctors move faster when they see history. Carry recent blood tests, ECG and echo reports, scan reports with CDs, previous discharge summaries and an allergy note. If records are stuck at another hospital, learn how to get medical records from hospital with a simple written request. A phone with an up-to-date emergency medical ID on phone also helps when the patient can't speak.

Personal items to pack

•      Two days of the patient's regular medicines in original strips

•      Glasses, hearing aids and dentures, labelled

•      Phone charger and a written list of family phone numbers

•      Comfortable clothes and slippers

Cashless hospitalisation documents and the pre-authorisation form

For insured patients, the documents required for hospital admission include a few extra papers. Cashless treatment means the insurer pays the hospital directly. It works only at network hospitals and only after the insurer approves a pre-authorisation request.

The cashless process, step by step
  1. Check the hospital is in your insurer's or TPA's network.

  2. At the TPA or insurance desk, show the e-card, policy number and patient ID.

  3. The hospital fills the pre-authorisation form for health insurance, with the doctor's diagnosis and cost estimate.

  4. For planned admissions, submit it 2 to 3 days in advance. For emergencies, the hospital sends it after admission.

  5. The insurer approves, partly approves or asks questions. Keep copies of every message.

  6. At discharge, the hospital sends the final bill for final authorisation.

Your rights under IRDAI's 2024 master circular

In May 2024, the insurance regulator IRDAI issued a master circular on health insurance. It asks insurers to decide on cashless authorisation requests within one hour, and on final discharge authorisation within three hours of the hospital's request. If the delay goes beyond three hours, any extra hospital charges are to be borne by the insurer. Policyholders are not required to submit claim documents; insurers and TPAs should collect them from the hospital.

Ayushman card hospital admission

Under Ayushman Bharat PM-JAY, eligible families get cover of up to ₹5 lakh per family per year at empanelled hospitals. All citizens aged 70 and above can get the Ayushman Vay Vandana card. Carry the Ayushman card or Aadhaar, and visit the Ayushman Mitra desk at the hospital. Check empanelled hospitals on the PM-JAY website or call 14555.

In simple terms: cashless means your insurance pays the hospital, so you don't have to. The hospital asks your insurer for approval, and the rules say insurers must answer quickly. Keep your insurance card and ID in your admission folder so this starts right away.

Before you leave: discharge documents to collect

The admission folder should come home fuller than it went in. These papers matter for follow-up care, claims and future treatment.

Collect before walking out

•      Discharge summary with diagnosis, treatment and follow-up plan

•      All investigation reports and imaging CDs

•      Itemised final bill and payment receipts

•      Prescription with doses and duration

•      Operation notes and implant stickers, if surgery was done

Planned surgery: what changes

For a planned surgery, the documents required for hospital admission also include pre-operative test reports (CBC, sugar, ECG, chest X-ray), the anaesthesia fitness note and a signed consent form. Many hospitals ask you to stop blood thinners days before, so bring the doctor's written instructions. Confirm the room category your policy covers, because room rent limits can affect the whole bill.

Claims and follow-up

For reimbursement claims, insurers usually ask for original bills, the discharge summary, reports and a claim form within the deadline in your policy. If you plan to see another specialist, these same files are what a second medical opinion in India depends on. Couples starting a family can store wedding-time results from a premarital health checkup in the same place, so maternity admissions have the background ready.

Seht tip

Photograph the documents required for hospital admission into each family member's Seht profile. At 2 a.m., you won't be searching for a policy number; it will be on your phone.

When to see a doctor

Don't wait for paperwork. Go to emergency or call for help if a family member has:

•      Chest pain, breathlessness or sweating with discomfort

•      Sudden weakness, numbness or slurred speech

•      High fever with drowsiness, confusion or a rash

•      Severe abdominal pain or persistent vomiting

•      Heavy bleeding or a serious injury

•      Very high or very low blood sugar with confusion

In a medical emergency, call 112 or 108. Hospitals must not delay emergency treatment while paperwork is arranged.

FAQs

What are the documents required for hospital admission in India?

You need the patient's photo ID, the attendant's ID, the doctor's admission note, past medical records and a current medicine list. For cashless treatment, add the health insurance e-card and policy number. For government schemes, carry the Ayushman card or ABHA. A deposit may be needed at private hospitals without cashless approval. Keep copies ready in one folder.

No single ID is mandatory for admission, and emergency care should never be refused for missing documents. Hospitals accept Aadhaar, PAN, passport, voter ID or driving licence for registration. Aadhaar is commonly used for Ayushman Bharat PM-JAY verification. Carry at least one photo ID for both the patient and the attendant.

Carry the insurance e-card, policy number, the patient's photo ID, the attendant's ID and the doctor's admission advice. The hospital's TPA desk fills the pre-authorisation form. Under IRDAI's 2024 master circular, insurers should decide cashless requests within one hour and discharge authorisation within three hours of the hospital's request. Keep the insurer's helpline handy.

Hospitals should provide emergency stabilising care first, and documents can be arranged later. The Supreme Court has held that emergency medical care cannot be delayed for formalities. If a private hospital asks for a deposit in a true emergency, ask for the medical superintendent, and call 112 if needed. Keep the receipt for any deposit you pay.

No. The Ayushman card works at hospitals empanelled under PM-JAY or your state's linked scheme. Many large private and most government hospitals are empanelled. Check the list on the PM-JAY website or by calling 14555 before a planned admission, and go to the Ayushman Mitra desk on arrival. Carry Aadhaar as well.
