This blood test is destroying Indians silently — And most don't know it exists

You got your checkup done. Everything came back 'normal.' You felt reassured.

What the report did not tell you: if your HbA1c, Lp(a), and homocysteine were never tested, you know almost nothing about your actual health risk.

This is happening to millions of Indians every year. A comprehensive-sounding package, a green checkmark, and no idea that the most dangerous conditions are progressing undetected.

Why 'Normal' checkup results are misleading millions of Indians

Standard basic panels in India CBC, lipid panel, fasting glucose, liver function, kidney function were designed primarily to detect acute illness and gross abnormalities. They do a reasonable job at that.

What they miss:

  • Silent prediabetes — Fasting glucose alone misses approximately 30% of prediabetes cases

  • Genetic cardiac markers like Lp(a) that are not included in any standard lipid panel

  • Nutritional deficiencies (Vitamin D, B12) that are near-epidemic in India but never tested

  • Homocysteine — A cardiovascular risk marker elevated in Indians on vegetarian diets

The result: Millions of Indians get a 'normal' report and feel falsely protected while silent damage continues.

The HbA1c test — The most important number most indians have never tested

HbA1c measures your 3-month average blood sugar by analyzing how much glucose has attached to your red blood cells. It cannot be manipulated by fasting before the test and gives a far more accurate picture of blood sugar control than fasting glucose alone.

Why it matters specifically for Indians:

  • India has 77 million diabetics and an estimated 130 million people with prediabetes most undiagnosed

  • Vascular damage begins at prediabetes levels, not just at diabetes diagnosis

  • A fasting glucose of 98 mg/dL looks fine, but an HbA1c of 6.1% reveals 3 months of borderline damage

India-specific ranges: Normal below 5.7% | Prediabetes 5.7–6.4% | Diabetes 6.5% and above.

Cost: Rs. 180–400 at Thyrocare or Dr. Lal Path Labs with home collection. This is the single highest-ROI blood test for most Indians.

Vitamin D3 and B12 — The deficiency epidemic nobody is talking about

Over 75% of Indians are Vitamin D deficient despite living in one of the sunniest countries in the world. The paradox exists because:

  • Darker skin pigmentation requires significantly more sun exposure to produce equivalent Vitamin D

  • Urban Indians spend most daylight hours indoors

  • Dietary sources of Vitamin D are limited even for non-vegetarians

The consequences of Vitamin D deficiency are not dramatic they are slow and cumulative: bone density loss, immune dysfunction, worsening insulin resistance, depression, muscle fatigue.

Vitamin B12 deficiency is near-universal in Indian vegetarians and also very common in metformin users (a first-line diabetes medication). B12 deficiency causes peripheral nerve damage that tingling or numbness in the feet many people attribute to 'circulation problems'. Left untreated, it causes irreversible nerve damage.

Both tests cost Rs. 400–800 each and are available with home collection.

Lipoprotein(a) — The genetic cardiac marker most doctors don't order

Lp(a) is a lipoprotein particle that is determined almost entirely by genetics no diet or exercise program will move it meaningfully. It is elevated in a disproportionate number of Indians and is an independent risk factor for heart disease and stroke.

The critical problem: it is not included in any standard lipid panel. Unless you or your doctor specifically requests it, you will never know.

If your Lp(a) comes back elevated (above 50 mg/dL is considered high risk), it changes your entire risk profile and your doctor's treatment approach more aggressive statin therapy, closer monitoring, and potentially newer medications.

Cost: Rs. 800–1,200. Ask for it by name.

How to Get These Tests and What to Do With the Results

These tests can all be ordered individually or bundled:

  • Thyrocare home collection: available in most cities, books via app or website

  • Dr. Lal PathLabs: India-wide, reliable, NABL accredited

  • 1mg and PharmEasy both offer home collection for diagnostics

When results come in:

  • Do not compare to the 'reference range' on the lab report without context reference ranges are often based on Western population averages

  • Bring results to your doctor with specific questions: 'Is my Lp(a) elevated? What does this HbA1c mean for me as an Indian?'

  • Store all reports in Seht to track trends over years a single reading is information, a multi-year trend is a health story

When to see a doctor

Health metrics: HbA1c ≥ 5.7%, Lp(a) > 50 mg/dL, Vitamin D < 20 ng/, B12 < 200 pg/mL, Homocysteine > 15. Recommendations included. Tracked on Seht

  • HbA1c at or above 5.7% — warrants a conversation about prevention

  • Lp(a) above 50 mg/dL — requires cardiovascular risk reassessment

  • Vitamin D below 20 ng/mL — supplementation needed, follow-up in 3 months

  • B12 below 200 pg/mL — urgent supplementation, evaluate cause

  • Homocysteine above 15 micromol/L — B-vitamin deficiency workup and dietary review

In simple terms

Most 'normal' checkup results in India are misleading not because the tests are wrong, but because the tests being run are not the right ones. This article explains the five tests that actually predict India's top killers.

FAQs

Why doesn't my doctor automatically order HbA1c?

Most standard health packages in India were designed decades ago and haven't kept pace with the changing disease profile. Additionally, HbA1c adds cost to a package. Ask specifically for it or build your own panel. Thyrocare allows you to order individual tests without a prescription.

Can I test for Lp(a) at home?

No home test exists for Lp(a) currently. You need a blood draw at a lab or home collection from a diagnostics company. The cost of Rs. 800–1,200 is reasonable given what it reveals.

How often should I test HbA1c?

If your HbA1c is in the normal range and you have no risk factors, once a year is sufficient. If you are in the prediabetes range (5.7–6.4%), test every 3–6 months and actively work on prevention. With diabetes, your doctor will recommend every 3 months.

My Vitamin D was low but I spend time in the sun. How is that possible?

Vitamin D production from sunlight depends on skin tone, time of day, latitude, and duration. In India, skin pigmentation requires 3–5× more UV exposure than lighter skin for equivalent production. Additionally, most sun exposure in India happens through glass (car, office) which blocks the UVB rays needed for synthesis.
